LEAVE OF ABSENCE
Versiti Leave of Absence and Return-to-Work Program provides employees across the organization with leaves of absence that support their needs when facing challenging life situations, either for themselves or a qualified family member. We recognize life situations can happen to anyone.
Versiti understands that all our employees are professionals committed to our mission, our donors, our patients, our coworkers, our families, and ourselves. Balancing those responsibilities is not an easy task, and, on some occasions, we might need some time off. The new policies provide employees with the opportunity to take time off in a job-protected leave under certain circumstances covered under these policies.
Versiti provides all employees with job-protected time away under certain situations. Our approach provides policies focused on the employee needs that allow for the coordination of legally-required and Versiti voluntary benefits.
Versiti also has a Return-to-Work Policy that facilitates a prompt return to work when the employee receives a medical release with or without restrictions. When we facilitate employees’ return to work, it is good for everyone. As an employee, you will have the opportunity to earn your full salary sooner, while Versiti benefits from having a trained employee back to work. Studies confirm that employees that stay on leave of absence for more than six months might not return to work, creating significant financial and emotional hardship for themselves and their families. Versiti strides to bring our employees back to work when a medical release allows them.

Your responsibility as an employee?
In case of absence:
〉 All employees MUST coordinate their time-off 30-days in advance for all planned absences, but never less than three days after the absence. This rule applies to both consecutive and intermittent leaves of absence.
〉 You must also contact The Hartford to register any absence that extends for more than three days, consecutive or intermittent.
〉 Timely provide medical documentation to The Hartford to process the Leave of Absence
In my return to work:
〉 Provide the return-to-work medical release immediately to your supervisor and human resources at HR4You@versiti.org
〉 Collaborate in conversations to determine a restricted duty assignment, if necessary
〉 Participate in restricted duty, modified work, or assigned tasks, as part of the restricted duty
〉 Participate in follow-up conversations and medical reviews, as needed, to determine progress in the condition and effectiveness of the assignments

If you are absent or expect to be absent from work in excess of 3 consecutive work days due to sickness or pregnancy or for an accidental injury you must report your absence by:

When you report your absence, you may need to verify or provide the following information:
〉 Personal Information
〉 Job Information
〉 Sickness/Injury Information (if applicable) – last day worked, nature of the illness, how, when, and where the injury occurred, and when disability commenced
〉 Physician Information (if applicable)– name, address, telephone number, and fax number for each treating physician
〉 Treatment provider Information (if applicable)–Name, address, telephone number, and fax number for each treating Health Care Provider.
〉 Authorization to Release Your Medical Information (if applicable):
- You should inform your Health Care Provider(s) that MetLife will be administering your disability claim and that you authorize the release of your medical information to the MetLife claims office.
- An “Authorization to Disclose Information About Me” form may be mailed to you from MetLife after you report your disability claim. You can expedite this process by downloading this form from MyBenefits at www.metlife.com/mybenefits. Click on the “Forms” link in the upper right hand corner of the navigation bar. You should sign and return this form as soon as possible. This release authorization will expedite the processing of your disability claim.
When you report a disability claim MetLife will send you written acknowledgement of your request. You may be contacted by a MetLife Case Manager within a few business days if we need to discuss additional information with you.

〉 You may be contacted to discuss your medical condition, including the impact it has on your ability to do your job, and your treatment plan.
〉 Your Health Care Provider may also be contacted, if applicable to discuss your medical information, treatment plan, prognosis and functional abilities.
〉 Regions may be contacted to discuss your specific job duties in detail. Confidential medical information will not be shared with Regions except for plan administration purposes such as return to work purposes.
Effective communication is a two-way process; therefore, you are encouraged to call your Case Manager anytime you have questions or concerns about the program or your case.
